Description

Problems that are corrected only at the surface tend to return, creating rework, delays, customer frustration, safety concerns, and audit findings. Root Cause Analysis – Fundamentals gives participants a practical way to investigate problems, organize evidence, identify likely causes, and choose actions that make recurrence less likely.

Throughout the course, participants learn how to:

  • Distinguish between symptoms, causes, and root causes.
  • Write clearer problem statements and gather useful evidence.
  • Use brainstorming, 5 Whys, Fishbone diagrams, and Pareto analysis appropriately.
  • Prioritize likely causes using evidence, risk, frequency, and severity.
  • Turn verified causes into corrective actions with owners, tracking, and effectiveness review.
  • Connect RCA results to continual improvement and the management system.

The course is designed for professionals who need a practical, structured approach to solving recurring problems in manufacturing, service, oil and gas, office, quality, safety, environmental, and operational settings.

Learning Objectives​

By the end of this course, participants will be able to:

  • Explain the purpose of Root Cause Analysis and when it should be used.
  • Distinguish between symptoms, problems, causes, and root causes.
  • Develop clear problem statements that keep the investigation focused and factual.
  • Collect and organize evidence from records, interviews, observations, logs, and other data sources.
  • Select and apply basic RCA tools, including brainstorming, 5 Whys, Fishbone diagrams, and Pareto analysis.
  • Recognize common RCA mistakes such as stopping too early, blaming people, or jumping to solutions.
  • Develop corrective actions that address verified causes and support recurrence prevention.
  • Explain how action tracking, effectiveness review, lessons learned, and RCA culture support continual improvement.

Prerequisites​

No prior Root Cause Analysis experience is required. Participants should be familiar with their organization’s processes, recurring problems, corrective action activities, or improvement efforts so they can connect the tools to real work situations.

Registration Transfer Policy

Registrations can be transferred to other participants or to other courses provided by Mireaux Management Solutions 30 days prior to the course start date.

  • Registration Transfers must be submitted in writing to [email protected].
  • There will be a 15% administrative fee applied to each registration transfer.
  • Payment for any difference in price, including the 15% administrative fee, will be required at the time of transfer to other courses of equal or greater value of the original paid amount.
  • Registration Transfers to a course of lesser value are allowed but are not eligible for refunds.
  • Registration Transfers are not allowed within 30 days of the course start date.

Cancellation Policy

Cancellations must be submitted in writing to [email protected] 30 days prior to the course start date in order to receive a refund, minus a 15% administrative fee. Cancellations are not allowed within 30 days of the course start date.
